Can Mounjaro Cause Muscle Pain?
Muscle pain is not listed as a common or very common side effect of Mounjaro (tirzepatide) in clinical trials or the UK prescribing information. However, some people taking weight loss injections report general aches, fatigue, or muscle-related discomfort. This guide explains what the evidence shows, when to contact your GP, and how to distinguish between normal adjustment effects and something that needs medical attention.

What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
What Does the Clinical Evidence Show?
Mounjaro clinical trials (the SURMOUNT studies) did not identify muscle pain as a frequent side effect. The most commonly reported side effects were gastrointestinal: nausea, diarrhoea, vomiting, and constipation. However, fatigue and general malaise were reported by some participants, which could contribute to a feeling of achiness or muscle discomfort.
If you experience persistent or severe muscle pain after starting Mounjaro, it is important to:
- Keep a symptom diary to identify patterns (timing, severity, which muscles)
- Note whether the pain coincides with nausea or stomach upset
- Contact your GP to rule out other causes (infection, overexertion, medication interactions)
- Inform your prescriber if the pain started immediately after an injection or dose increase
Muscle pain lasting more than a few days or worsening over time should always be discussed with your GP rather than assumed to be a side effect of Mounjaro.

When to Seek Medical Advice

- Severe, persistent abdominal pain (possible pancreatitis — a serious but rare condition)
- Unexplained weakness or difficulty moving
- Chest pain or difficulty breathing
- Fever or signs of infection
- Rapid heart rate or palpitations
Routine muscle aches not linked to these symptoms can usually be discussed with your GP at a regular appointment. If you are taking Mounjaro as part of a weight management programme, your prescriber should be informed of any new or concerning symptoms. Use our free BMI calculator and GLP-1 weight loss calculator to track your overall progress and discuss any concerns at your next clinical review.

Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
Is muscle pain a known side effect of Mounjaro in the UK?
Muscle pain is not listed as a common or very common side effect in UK clinical trials or prescribing information. Fatigue and general malaise were reported by some participants, but specific muscle pain was uncommon. If you experience persistent muscle pain, contact your GP.
Could muscle pain be related to weight loss itself rather than Mounjaro?
Yes. gradual weight management, changes in physical activity, or increased exercise can cause muscle soreness independently of the medication. Dehydration is also common with Mounjaro's gastrointestinal side effects and can cause muscle cramping. Ensure you drink enough water.
What is the difference between Mounjaro (tirzepatide) and Wegovy (semaglutide)?
Mounjaro is a dual GIP and GLP-1 receptor agonist; Wegovy is a GLP-1 agonist only. Both are prescription-only weight management injections. Mounjaro typically has a longer onset and different side effect profile. Muscle pain is not commonly reported with either.
Should I stop taking Mounjaro if I develop muscle pain?
Do not stop without medical advice. Contact your GP or prescriber first. Muscle pain could be unrelated to Mounjaro, and stopping without guidance may affect your weight management plan. Your GP can assess the cause and advise next steps.
How can I reduce muscle aches if I'm taking Mounjaro?
Stay well hydrated, maintain gentle physical activity, ensure adequate sleep, and manage nausea (which can contribute to general discomfort). If you're new to Mounjaro, some adjustment effects settle within a few weeks. Keep your GP informed of any persistent symptoms.
