Can You Take Mounjaro If You Don't Have a Gallbladder?

If you've had your gallbladder removed (cholecystectomy) and are considering Mounjaro for weight management, you may wonder whether this GLP-1 medication is still safe for you. The straightforward answer is that having no gallbladder doesn't automatically disqualify you from Mounjaro or Wegovy, but your prescriber needs to know this during your clinical assessment. Both medications can affect digestion and bile flow, so individual suitability requires professional evaluation. This guide explains what you need to know about taking prescription weight loss injections after gallbladder removal.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

Mounjaro, Wegovy, and Gallbladder Health — Clinical Considerations

Mounjaro (tirzepatide) is a prescription-only, once-weekly injectable medication that works as a dual GIP and GLP-1 receptor agonist. Wegovy (semaglutide) is a once-weekly GLP-1 agonist. Both can slow gastric emptying—how quickly food moves through your stomach—which affects bile production and digestion. If you've had a cholecystectomy, your body has already adapted to digest fats without a gallbladder by releasing bile continuously from the liver rather than in concentrated amounts. However, GLP-1 medications may further slow this process, potentially causing digestive discomfort.

In clinical trials: SURMOUNT-1 (Mounjaro) showed average weight loss of 20.9% at the maximum 15mg dose, whilst STEP-1 (Wegovy) showed clinically studied weight loss on the 2.4mg dose. Neither trial specifically excluded people without gallbladders, but post-cholecystectomy patients were likely under-represented. Your prescriber needs to assess whether either medication is right for you individually, using your full medical history.

What Your Prescriber Will Consider

Before prescribing Mounjaro or Wegovy, your clinician will review your complete health profile, including previous gallbladder removal and any ongoing digestive symptoms. If you experience persistent diarrhoea, fatty stool (steatorrhoea), or abdominal discomfort after cholecystectomy, this information is crucial. Some people report improved digestion on GLP-1 medications because reduced appetite and slower eating can ease post-cholecystectomy symptoms; others may experience worsening nausea or constipation.

Your prescriber may recommend starting at a lower dose and increasing gradually to monitor how your body responds. You should also use our free GLP-1 weight loss calculator to explore expected outcomes at different doses, and our BMI calculator to establish your baseline health metrics before beginning treatment. Always inform your GP or prescriber about your cholecystectomy and any related digestive issues.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Is Mounjaro safe if I don't have a gallbladder?

Mounjaro is not automatically unsafe without a gallbladder, but your prescriber must know about your cholecystectomy during your clinical assessment. GLP-1 medications affect digestion and bile flow, so individual suitability varies. Your GP or prescriber will evaluate your specific circumstances and any ongoing digestive symptoms before deciding whether Mounjaro is appropriate for you.

How does Mounjaro differ from Wegovy in terms of gallbladder health?

Mounjaro is a dual GIP and GLP-1 agonist, whilst Wegovy is a GLP-1 agonist only. Both slow gastric emptying and can affect digestion. There is no clinical evidence that one is safer than the other for post-cholecystectomy patients. Your prescriber will base their choice on your overall health profile, weight loss goals, and how your body typically responds to medication.

What symptoms should I watch for if I take Mounjaro without a gallbladder?

Common Mounjaro side effects include nausea, diarrhoea, constipation, and abdominal pain. If you've had your gallbladder removed, watch for worsening fatty stools, persistent diarrhoea, or abdominal discomfort that differs from your normal post-surgery digestion pattern. Report any concerning changes to your GP immediately, especially severe stomach pain.

Can I switch from Mounjaro to Wegovy if I develop gallbladder-related side effects?

Switching medications is possible, but only under prescriber guidance. Both Mounjaro and Wegovy affect digestion similarly, so switching alone may not solve gallbladder-related issues. Your prescriber might adjust your dose, recommend dietary changes, or explore whether the medication is suitable long-term. Never stop or switch medications without professional advice.

Should I mention my cholecystectomy when I apply for Mounjaro?

Yes, absolutely. When you apply for Mounjaro or Wegovy through an online clinical assessment, you must disclose that you've had your gallbladder removed. This is essential information for the prescriber to assess your eligibility. Failing to mention it could lead to inappropriate prescribing and preventable side effects.
