Does Mounjaro Increase Blood Pressure?
Blood pressure concerns are common when considering GLP-1 or dual GIP/GLP-1 weight loss medications. The evidence suggests that Mounjaro (tirzepatide) does not directly increase blood pressure in most patients — in fact, weight loss itself typically improves blood pressure over time. However, individual responses vary, and any medication can affect people differently. This guide compares Mounjaro and Wegovy in terms of cardiovascular effects, side effects, and what NHS guidance says about monitoring.

What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
Blood Pressure Effects: Mounjaro vs Wegovy — Clinical Evidence
Mounjaro (tirzepatide) clinical trials showed no significant increase in blood pressure. In the SURMOUNT-1 trial, participants on Mounjaro 15 mg actually experienced modest improvements in systolic and diastolic blood pressure as they lost weight — an average reduction of approximately 2–3 mmHg. This is expected because weight loss itself reduces hypertension risk.
Wegovy (semaglutide) similarly showed no direct blood pressure increase in STEP trials. Both medications may cause tachycardia (increased heart rate) in a small number of patients, particularly during dose escalation, but this is uncommon and typically mild.
- Weight loss from either medication usually improves blood pressure control
- Patients with existing hypertension can safely use both medications with GP monitoring
- Heart rate changes are rare and usually temporary
- Both require baseline cardiovascular assessment before starting
The dual GIP/GLP-1 mechanism of Mounjaro does not elevate blood pressure differently than the single GLP-1 mechanism of Wegovy — individual health profiles matter more than the medication type.

Individual Suitability and NHS Monitoring
Whether Mounjaro or Wegovy is appropriate depends on your personal health history, not on blood pressure effects alone. Both are prescription-only medications requiring clinical assessment by a UK-licensed prescriber. If you have hypertension, your GP or prescriber will want to monitor your blood pressure regularly — not because the medication raises it, but to track the positive effect of weight loss.
Patients taking blood pressure medications may need dose adjustments as they lose weight and their hypertension improves. Your prescriber will manage this. Use our free GLP-1 weight loss calculator to estimate potential weight loss, and discuss any cardiovascular concerns with your GP before starting either medication. Both Mounjaro via tirzepatide dosing and Wegovy require baseline assessment and ongoing monitoring.

How We Built These Tools
Every calculator and guide on myweightcheck is built with accuracy and transparency in mind. Our BMI calculator uses the standard WHO formula with NHS-aligned category thresholds. Our GLP-1 projections are modelled on published clinical trial data from SURMOUNT and STEP studies.
All data stays on your device. We don't require sign-ups, and we don't store your health information on any server. Your progress tracker saves data locally in your browser using localStorage, so only you can see it.
We're based in the UK and our tools are designed specifically for UK users — supporting stone & lbs by default, referencing NHS guidelines, using NICE-approved medication data, and linking to GPhC-registered pharmacies.
Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
Does Mounjaro directly raise blood pressure?
No. Clinical trials (SURMOUNT) showed no blood pressure increase from Mounjaro. Most patients experienced slight improvements due to weight loss. Rare cases of mild heart rate increase have been reported but are not dangerous or permanent.
Can I take Mounjaro if I have high blood pressure?
Yes, if you have been assessed as suitable by a prescriber. In fact, Mounjaro may help improve blood pressure control through weight loss. Your GP should monitor your blood pressure regularly and adjust any existing hypertension medications if needed.
How does Mounjaro compare to Wegovy for blood pressure effects?
Both Mounjaro and Wegovy have similar cardiovascular safety profiles. Neither directly increases blood pressure. The difference between them (dual GIP/GLP-1 vs single GLP-1) does not significantly affect blood pressure outcomes — individual health factors matter more.
What should I monitor if I'm on Mounjaro and have hypertension?
Your prescriber will advise regular blood pressure monitoring. As you lose weight, your hypertension may improve, potentially requiring adjustment of blood pressure medications. Report any chest pain, unusual shortness of breath, or significant heart rate changes to your GP immediately.
Is there a difference in heart rate effects between Mounjaro and Wegovy?
Both medications can cause mild, temporary heart rate increases during dose escalation in a small percentage of users. This is rare and typically resolves. Serious cardiac effects are not associated with either medication in clinical trials. Your baseline health assessment identifies any specific cardiac risks.
