Why Am I Still Hungry On Mounjaro?

If you're taking Mounjaro and still experiencing hunger, you're not alone — appetite suppression varies significantly between individuals. Both Mounjaro (tirzepatide) and Wegovy (semaglutide) are designed to reduce hunger signals, but some people report feeling less appetite suppression than expected. This can be due to dose, individual physiology, or lifestyle factors. Understanding how these medications work and comparing them can help you discuss your experience with your prescriber. Use our free GLP-1 weight loss calculator to track how your appetite and progress respond to your current dose.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

How Mounjaro and Wegovy Differ in Appetite Suppression

Mounjaro is a dual GIP and GLP-1 receptor agonist, meaning it activates two separate hormone pathways that regulate appetite and blood sugar. This dual mechanism may theoretically provide stronger appetite suppression than single-pathway medications. Wegovy, by contrast, is a GLP-1-only agonist that works through a single hormone pathway.

Clinical trial data shows different appetite profiles: SURMOUNT-1 trial data (Mounjaro at 15 mg) demonstrated an average weight loss of approximately 20.9% of body weight over 72 weeks. STEP-1 trial data (Wegovy at 2.4 mg) showed approximately 14.9% average weight loss over 68 weeks. However, average outcomes mask individual variation — some people on Mounjaro report significantly reduced hunger, whilst others feel minimal appetite suppression even at maximum dose.

Dose escalation matters: Mounjaro starts at 2.5 mg and escalates weekly to a maximum of 15 mg over several months. Your prescriber may adjust your dose if you're not experiencing adequate appetite suppression. If you're still hungry, dose titration (gradual increase) is often the first step, rather than switching medications.

When to Discuss Persistent Hunger With Your Prescriber

Feeling hungry on Mounjaro doesn't mean the medication isn't working — weight loss can still occur through other mechanisms, including improved blood sugar regulation and reduced overall food intake even without dramatic appetite suppression. However, appetite reduction is an expected benefit, and persistent hunger may warrant a prescriber review.

Contact your GP or specialist weight management service if:

  • You're on Mounjaro 15 mg (maximum dose) and experiencing no appetite suppression after 4+ weeks at that dose
  • You're struggling to stick to a reduced-calorie diet despite taking the medication
  • You experience new or worsening hunger alongside other side effects
  • Your weight loss has plateaued despite good dietary adherence

Your prescriber may consider dose optimisation, lifestyle adjustments, or discussing whether a medication switch (such as to Wegovy or another option) might be more suitable. Every person's response is different.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Is it normal to feel hungry on Mounjaro?

Yes. Whilst Mounjaro is designed to suppress appetite, individual responses vary widely. Some people feel significantly reduced hunger; others experience mild or inconsistent appetite suppression. Genetics, baseline metabolism, dose, and lifestyle all influence how pronounced the effect is. If you're on a stable dose and still hungry, discuss this with your prescriber.

How does Mounjaro's dual GIP and GLP-1 mechanism affect hunger compared to Wegovy's single GLP-1 pathway?

Mounjaro's dual mechanism may theoretically provide stronger appetite suppression than single-pathway GLP-1 medications like Wegovy. However, clinical trials show individual variation is significant — some people respond better to one mechanism than the other. This is why prescriber assessment and dose optimisation are personalised.

Should I switch from Mounjaro to Wegovy if I'm still hungry?

Not necessarily immediately. First, discuss with your prescriber whether dose escalation, lifestyle changes, or other optimisations might help. Some people feel more appetite suppression at higher Mounjaro doses. A medication switch is a clinical decision made with your prescriber based on your full health profile, not hunger alone.

Can I take a higher Mounjaro dose if I'm hungry?

Maximum Mounjaro dose is 15 mg once weekly. If you're below this, your prescriber may increase your dose in 2.5 mg increments. However, higher doses don't assures increased appetite suppression — individual response varies. Your prescriber will consider your full clinical picture, including any side effects.

Does persistent hunger mean Mounjaro isn't working for weight loss?

No. Weight loss occurs through multiple mechanisms, not just appetite suppression — including improved blood sugar regulation, altered fat utilisation, and reduced overall calorie intake. Some people lose weight steadily without dramatic hunger reduction. Track your progress using our free progress tracker and discuss weight loss trajectory (not just hunger) with your prescriber.
