Mounjaro 2.5 mg Dose In Millilitres — What You Need to Know

The 2.5 mg starting dose of Mounjaro (tirzepatide) comes as a pre-filled KwikPen injection containing 0.5 ml of liquid. Each KwikPen holds four weekly doses, so one pen supplies a full month of treatment at the starting dose. This introductory dose is designed to help your body adjust to the medication before gradually increasing to higher doses. Understanding the volume and delivery method helps you feel confident when you first start taking Mounjaro as part of your weight management journey with prescription support.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

How Mounjaro 2.5 mg Is Delivered

Mounjaro comes in a pre-filled KwikPen auto-injection device, not a traditional syringe. Each pen contains enough medication for four weekly injections, meaning one KwikPen lasts exactly one month. The 2.5 mg starting dose is administered as a 0.5 ml subcutaneous injection — injected just under the skin on your stomach, thigh, or upper arm.

  • Each KwikPen contains 4 doses of 2.5 mg (0.5 ml per injection)
  • The needle is pre-set — you don't select the dose yourself
  • Injection takes just a few seconds
  • KwikPens are stored in the refrigerator before use (2–8°C)
  • After first use, can be kept at room temperature for up to 30 days

The 2.5 mg dose is purely a starting dose. After 4 weeks, your prescriber will typically increase you to 5 mg, then continue escalating by 2.5 mg increments every 4 weeks until you reach an effective maintenance dose (usually 10 mg or 15 mg).

Starting Mounjaro — What to Expect at 2.5 mg

At the 2.5 mg starting dose, Mounjaro is gentler on your digestive system than higher doses, though some people experience mild nausea, appetite changes, or stomach adjustments in the first week or two. These effects often ease as your body adapts.

Your GP or prescriber will monitor how well you tolerate this dose before increasing. Try our free GLP-1 weight loss calculator to estimate potential results at different doses, and use our BMI calculator to track your baseline before starting treatment. Keep a record of any side effects using our progress tracker — this helps your prescriber decide when and how to escalate your dose safely.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Is 2.5 mg the highest dose of Mounjaro?

No. 2.5 mg is the starting dose only. Mounjaro is gradually increased every 4 weeks: 2.5 mg → 5 mg → 7.5 mg → 10 mg → 12.5 mg → 15 mg (maximum). Most people reach an effective dose of 10 mg or 15 mg within 4–5 months.

How much does a 2.5 mg KwikPen cost in the UK?

Mounjaro is prescription-only and costs vary by provider. Private costs typically range from £130–£180 per KwikPen (one month supply) depending on your prescriber. NHS access is available through specialist weight management services, though availability and waiting times vary significantly by region.

Can I inject Mounjaro myself with the 2.5 mg KwikPen?

Yes. The KwikPen is designed for self-injection. Your prescriber or pharmacist will show you how to inject safely before you start. You can inject into your stomach, thigh, or upper arm — rotate injection sites each week to avoid irritation.

What if I miss a 2.5 mg dose of Mounjaro?

If you miss your weekly injection, take it within 4 days and continue your normal schedule the following week. If more than 4 days have passed, skip that dose and resume on your normal injection day. Never take two doses close together — maintain at least 72 hours (3 days) between injections.

Is Mounjaro 2.5 mg the same as Wegovy?

No. Both are weight loss injections, but Mounjaro (tirzepatide) uses a dual GIP and GLP-1 mechanism, while Wegovy (semaglutide) is GLP-1 only. Mounjaro starts at 2.5 mg; Wegovy starts at 0.25 mg. Individual responses vary, and only your prescriber can recommend which is suitable for you.
