What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
NHS Eligibility and Mounjaro 5mg Prescribing
NICE guidance approved tirzepatide (Mounjaro) for use within NHS specialist weight management services in 2024. To access Mounjaro on the NHS, you must be referred to a tier 3 specialist weight management service by your GP — it is not available through standard primary care. Eligibility requires a BMI of 30 kg/m² or above, or 27–29.9 kg/m² with weight-related health conditions such as type 2 diabetes, hypertension, or cardiovascular disease. Adults of South Asian, Chinese, Black African, or Black Caribbean heritage may have lower BMI eligibility thresholds.
The 5mg dose is reached after 4 weeks on the 2.5mg starting dose. At this stage, clinical trials showed participants experienced meaningful appetite reduction and early weight loss. NHS availability varies significantly by region — some areas have long waiting lists, whilst others may have limited capacity. Your GP can discuss eligibility and referral options during your appointment.

NHS vs Private Access for Mounjaro 5mg
NHS route: Free through your GP and tier 3 services, but involves referral, assessment, and potentially long waiting times. MHRA monitoring (Black Triangle status) is managed through NHS systems. NHS prescribers will also monitor you for side effects and adjust doses as needed.
Private route: Available through GPhC-registered online pharmacies following an online clinical assessment with a UK-licensed prescriber. Faster access (typically days rather than months), but you pay for the medication — costs range from approximately £129–£180 for a 5mg KwikPen depending on provider. Both routes require a valid prescription. Use our BMI calculator above to check your personal eligibility, and our GLP-1 weight loss calculator to estimate potential outcomes at the 5mg dose.
