Mounjaro and Aching Legs — What You Need to Know
Muscle aches and leg pain are not listed as common side effects of Mounjaro (tirzepatide) in UK clinical trial data, but some patients report joint and muscle discomfort during treatment. If you're experiencing persistent aching legs whilst taking Mounjaro, it's important to understand whether this is related to the medication, an underlying condition, or another cause. This guide explains what the evidence shows, when to contact your GP, and how NHS services in the UK can help assess your symptoms. Use our free GLP-1 weight loss calculator to explore whether Mounjaro remains suitable for your situation, and always discuss any new symptoms with your healthcare provider.

What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
Mounjaro Side Effects and Muscle Pain — NHS Context
Muscle aches and joint pain are not listed as very common or common side effects of Mounjaro in the UK Product Information leaflet or MHRA safety data. The most frequently reported side effects during the SURMOUNT clinical trials were nausea, diarrhoea, vomiting, and constipation — gastrointestinal effects linked to how tirzepatide slows gastric emptying.
However, some patients taking GLP-1 and dual GIP/GLP-1 agonists report muscle soreness, generalised aches, or joint discomfort. This may occur due to several reasons:
- gradual weight management — fast reduction in body weight can place temporary stress on muscles and joints, particularly in the knees, hips, and lower back
- Changes in physical activity — increased exercise or changes in movement patterns during weight loss
- Dehydration — GLP-1 medications can reduce appetite for fluids as well as food; inadequate hydration may contribute to muscle cramps
- Electrolyte changes — significant dietary changes may affect sodium, potassium, or magnesium levels
- Unrelated conditions — arthritis, fibromyalgia, or other musculoskeletal conditions coinciding with Mounjaro treatment
If aching legs started after beginning Mounjaro and persist beyond a few weeks, your GP should be consulted to rule out other causes and assess whether the medication is appropriate for you.

When to Contact Your GP — NHS Guidance
Contact your GP if aching legs:
- Are severe or worsening despite rest and pain relief
- Started after beginning Mounjaro and are persistent (longer than 2–3 weeks)
- Are accompanied by swelling, redness, warmth, or signs of infection
- Limit your ability to walk, exercise, or carry out daily activities
- Are accompanied by other new symptoms such as fever, fatigue, or unexplained weight loss
Your GP can perform a physical examination, review your Mounjaro dose and timeline, assess whether dose titration is too rapid, and determine if the aching is related to your weight loss rate or an independent condition. If you're accessing Mounjaro through a private prescriber, contact them immediately with new symptoms — they have a duty to review your treatment safety. Do not stop taking Mounjaro without guidance from your prescriber.

Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
Is muscle pain a known side effect of Mounjaro in the UK?
Muscle aches and joint pain are not listed as very common or common side effects in UK safety data or the MHRA Product Information leaflet. The most common side effects are gastrointestinal (nausea, diarrhoea, vomiting). However, some patients report muscle discomfort, which may be linked to gradual weight management, dehydration, or unrelated conditions rather than the medication directly.
Could my aching legs be caused by weight loss rather than Mounjaro itself?
Yes. gradual weight management places temporary stress on muscles, joints, and connective tissue, especially in the knees, hips, and lower back. Mounjaro helps achieve significant weight loss — averaging 15–21% body weight in clinical trials — which can cause muscle soreness. Slower weight loss and adequate hydration may reduce this effect. Your GP can advise on safe weight loss rates.
What should I do if I develop aching legs whilst taking Mounjaro?
Contact your GP or prescriber if aching legs are severe, persistent (longer than 2–3 weeks), or limit your ability to walk and exercise. Your GP can assess whether the pain is related to your weight loss rate, dehydration, electrolyte changes, or an independent condition. Do not stop Mounjaro without guidance.
Could dehydration from Mounjaro cause muscle aches?
Possibly. GLP-1 medications like Mounjaro reduce appetite for both food and fluids. Dehydration can contribute to muscle cramps and aches. Ensure you drink plenty of water throughout the day, especially during dose titration. Aim for 6–8 glasses of water daily, or as recommended by your GP.
How does Mounjaro dose titration relate to muscle pain?
Mounjaro is increased gradually over 20+ weeks (starting at 2.5 mg, increasing to a maximum of 15 mg). Rapid dose increases can accelerate weight loss and may increase muscle soreness. If aching legs coincide with a dose increase, inform your prescriber — slowing titration may help. Your GP can assess whether your dose schedule is appropriate for you.
