Mounjaro and Appendicitis — What the Evidence Shows
Appendicitis is inflammation of the appendix, a small tube-shaped organ in the lower right abdomen. Some people taking Mounjaro (tirzepatide) have reported abdominal pain, raising questions about whether the medication increases appendicitis risk. This guide explains what clinical evidence shows about Mounjaro and appendicitis, how to recognise warning signs, and when to seek urgent medical help. Always consult your GP if you experience severe abdominal pain while taking any medication.

What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
What Clinical Evidence Shows About Mounjaro and Appendicitis Risk
Mounjaro is a dual GIP and GLP-1 receptor agonist approved in the UK for weight management in adults. During clinical trials (SURMOUNT studies), appendicitis was not identified as a significant safety concern. However, Mounjaro can cause gastrointestinal side effects including nausea, constipation, diarrhoea, and abdominal discomfort — which are separate from appendicitis risk.
The MHRA (UK medicines regulator) has not issued any specific warnings linking tirzepatide to increased appendicitis risk. Appendicitis itself is relatively rare in the general population (approximately 7 cases per 10,000 people per year in the UK). If appendicitis does occur, it is typically not causally linked to weight loss medication, but rather develops independently.
That said, because Mounjaro affects how the digestive system works by slowing gastric emptying, it is important to distinguish between common GLP-1 side effects and signs of a serious abdominal condition. Your prescriber will have assessed your suitability for this medication during your clinical assessment.

How to Recognise Appendicitis vs. Mounjaro Side Effects
Common Mounjaro side effects include mild to moderate stomach pain, nausea, and constipation that typically improve within days or weeks as your body adjusts. These usually resolve on their own or with dietary changes.
Appendicitis warning signs require urgent medical attention: severe, sharp pain in the lower right abdomen; pain that worsens over hours; fever; nausea and vomiting alongside the pain; inability to eat or drink. If you experience these symptoms, call your GP immediately or attend A&E. Do not wait to see if the pain resolves.
Always report new or worsening abdominal pain to your GP or prescriber. They can assess whether symptoms are related to Mounjaro adjustment or require further investigation. Your GP can examine you and, if necessary, arrange imaging (ultrasound or CT scan) to rule out appendicitis or other serious conditions.

Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
Does Mounjaro cause appendicitis?
No direct causal link has been established between tirzepatide (Mounjaro) and appendicitis risk. Clinical trials did not identify appendicitis as a safety concern. However, Mounjaro can cause abdominal discomfort as a common side effect, which may be confused with other abdominal conditions. If you experience severe abdominal pain, seek urgent medical advice.
What abdominal pain is normal when starting Mounjaro?
Mild to moderate stomach pain, often alongside nausea, is a very common side effect affecting more than 1 in 10 people. This typically improves within days or weeks as your body adjusts. Pain should be manageable and intermittent. Severe, persistent pain spreading to the back or accompanied by fever requires urgent medical assessment.
When should I go to A&E with abdominal pain on Mounjaro?
Seek urgent care if you experience: sudden severe pain in the lower right abdomen; pain worsening rapidly over hours; fever above 38°C; persistent vomiting; inability to keep food or fluids down; or pain unrelieved by rest. Do not assume it is a Mounjaro side effect — let medical professionals assess you.
Should I stop taking Mounjaro if I develop abdominal pain?
Do not stop without speaking to your prescriber first. Report the pain to your GP or the service that prescribed your Mounjaro. They can assess whether the pain is a known side effect, adjust your dose, or arrange tests to rule out other conditions. Never self-manage by stopping prescription medication.
Can I take Mounjaro if I have had appendicitis before?
Having had appendicitis removed in the past is not a contraindication to Mounjaro, as your appendix can no longer become inflamed. However, always disclose your full medical history to your prescriber during your clinical assessment. They will assess your suitability based on your individual health profile and any other GI conditions.
