How Mounjaro Affects Belly Fat — Evidence and Reality

Mounjaro (tirzepatide) is a prescription-only weight loss injection that may help reduce overall body fat, including belly fat — but results vary significantly between individuals. Clinical trials show that participants lost an average of 15–21% of their body weight when using Mounjaro alongside a reduced-calorie diet and increased physical activity. However, where your body loses fat first (including whether belly fat reduces) depends on genetics, starting weight, dose, and individual metabolism. This guide explains what the evidence actually shows and sets realistic expectations based on UK clinical data.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

What Clinical Trials Show About Mounjaro and Fat Loss

The SURMOUNT-1 clinical trial, which tested tirzepatide (Mounjaro's active ingredient) in adults with obesity, showed average weight loss of approximately 20.9% at the maximum dose (15 mg weekly) over 72 weeks. Importantly, this was total body weight loss — the trial did not specifically isolate belly fat or visceral fat reduction. Weight loss with Mounjaro works through dual GIP and GLP-1 receptor activation, which reduces appetite, increases fullness, and supports fat metabolism. Clinical trials showed participants lost fat alongside preserved muscle when they combined the injection with structured diet and exercise.

However, before and after comparisons are individual. Some people may notice belly fat reducing first; others lose from different areas. This is determined by your genetics and where your body naturally stores and mobilises fat. Additionally, gradual weight management — especially very rapid loss — can result in loose skin around the abdomen in some cases, which is an important realistic expectation. Your GP or prescriber can discuss your individual likely outcomes based on your starting point and health factors.

Belly Fat, Visceral Fat, and Health Risk

Belly fat is not just cosmetic — visceral fat (fat stored around organs in the abdomen) is metabolically active and linked to increased risk of type 2 diabetes, heart disease, and fatty liver disease. One reason weight loss injections like Mounjaro may be prescribed is specifically to reduce this health risk. Studies suggest that even modest weight loss (5–10% of body weight) can significantly reduce visceral fat and improve metabolic markers like blood sugar and cholesterol.

When considering Mounjaro, remember that weight loss happens gradually — typically 0.5–2 kg per week depending on starting weight and dose. Expecting dramatic overnight changes is unrealistic. Use our free BMI calculator to check your current health category, and try our GLP-1 weight loss calculator to see projected outcomes at different doses based on clinical trial data. Your prescriber will also discuss realistic timelines and progress expectations.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Do before and after photos show real Mounjaro results?

Before and after photos circulating online may represent real individual results, but they are not typical or assured. Clinical trials show average weight loss of 15–21%, but individual outcomes vary enormously based on starting weight, adherence, diet, exercise, and genetics. Also, photos can be misleading due to lighting, posture, and timing. Your GP can discuss realistic expectations for your situation.

How long before you notice belly fat reduction on Mounjaro?

Most people begin noticing weight loss within 4–8 weeks on an effective dose, though some notice changes sooner. Belly fat may reduce gradually throughout treatment, but the rate and timing vary. Weight loss typically progresses at 0.5–2 kg per week alongside dietary changes. Patience and consistency matter more than rapid results.

Does Mounjaro specifically target belly fat?

No — Mounjaro promotes overall body weight loss, not spot reduction. Your body decides where fat comes from during weight loss based on genetics. Some people lose belly fat first; others lose from hips, thighs, or other areas. The good news is that visceral (belly) fat is often among the first fat types mobilised during weight loss.

Can I get Mounjaro on the NHS for belly fat reduction?

Mounjaro is available through NHS specialist weight management services (tier 3) if you meet eligibility criteria — typically BMI 30 or above, or BMI 27+ with weight-related health conditions like type 2 diabetes or high blood pressure. Access varies significantly by region (ICB). Private prescription is also available through UK-licensed online prescribers.

How does Mounjaro compare to Wegovy for belly fat loss?

Mounjaro (tirzepatide — dual GIP and GLP-1) and Wegovy (semaglutide — GLP-1 only) both support weight loss. Clinical trials suggest Mounjaro may result in slightly higher average weight loss (up to 21% vs 15%), but individual results vary greatly. Both require prescription and clinical assessment. Your prescriber can discuss which is more suitable for you.
