Mounjaro and Shingles — What UK Patients Need to Know
If you're considering Mounjaro (tirzepatide) for weight management in the UK, you may have questions about how it affects your immune system and vaccine protection, including shingles risk. This guide explains the relationship between GLP-1 and GIP receptor agonists, immune function, and what NHS guidance says about shingles vaccination timing. Individual circumstances vary, and your GP can provide personalised advice based on your health profile.

What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
Mounjaro, Immune Function, and Shingles Risk
Mounjaro (tirzepatide) is a dual GIP and GLP-1 receptor agonist that works on appetite and glucose control — it is not an immunosuppressant medication. Unlike some weight loss or diabetes treatments, tirzepatide does not suppress the immune system, and there is no evidence that Mounjaro increases your risk of shingles or other infections at standard doses.
Shingles (herpes zoster) risk is primarily determined by:
- Age — risk increases significantly after age 50
- Immune system strength — which can be affected by stress, sleep, nutrition, and certain medical conditions
- Previous chickenpox exposure — required to develop shingles later in life
- Vaccination status — the shingles vaccine (Shingrix) offers strong protection
Clinical trials of Mounjaro have not identified shingles as a notable safety concern. However, weight loss itself can sometimes affect immune function if nutrition is poor, which is why balanced, supervised weight loss is recommended.

Mounjaro, Shingles Vaccination, and NHS Timing Guidance
If you are eligible for shingles vaccination under NHS guidance (typically age 65, or age 50+ with certain health conditions), this can be given at any point around starting Mounjaro. There is no requirement to delay or adjust vaccination timing because Mounjaro is not immunosuppressive.
Shingrix (the two-dose shingles vaccine used in the UK) is an inactivated vaccine — it does not contain live virus and is safe to give to people on Mounjaro. If you are due for shingles vaccination, discuss the timing with your GP or pharmacist at your weight management clinic. Vaccination is generally recommended before or alongside weight loss treatment, as maintaining strong immunity is part of overall health during weight loss.

Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
Does Mounjaro increase my risk of shingles?
No. Mounjaro is not an immunosuppressant and clinical trials have not identified increased shingles risk. Shingles risk depends on age, immune health, and vaccination status. If you have concerns about infection risk, discuss your individual health profile with your GP.
Can I have the shingles vaccine (Shingrix) while taking Mounjaro?
Yes. Shingrix is an inactivated vaccine and is safe to give alongside Mounjaro. There is no need to delay vaccination. Discuss timing with your GP or clinic — vaccination is often recommended as part of preventative care during weight loss treatment.
Does weight loss from Mounjaro weaken my immune system?
No, if weight loss is balanced and supervised. Very gradual weight management or poor nutrition can affect immune function, which is why NHS weight loss services recommend sustainable loss (0.5–1 kg/week) alongside adequate protein, vitamins, and minerals. A structured approach protects immune health.
What should I tell my GP about Mounjaro and shingles?
Mention that you are considering or taking Mounjaro as part of your weight management plan. Your GP can review your shingles vaccination status and recommend vaccination if you're eligible. Always discuss any new medications or health concerns with your prescriber or GP.
Is there any link between tirzepatide and shingles reactivation?
No established link. Tirzepatide does not reactivate dormant shingles virus. If you develop shingles symptoms while on Mounjaro, seek medical advice promptly — but the cause is unlikely to be the medication itself. Your GP can assess and provide appropriate treatment.
