Mounjaro (tirzepatide) is a prescription-only weight management injection licensed by the MHRA for UK adults. Whether you access it through the NHS or privately, you'll need a clinical assessment to check eligibility and ensure the medication is suitable for you. What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

NHS Mounjaro Consultation: Eligibility and Tier 3 Services

The NHS has approved tirzepatide (Mounjaro) for use within specialist weight management services, following NICE guidance. Access is through tier 3 or tier 4 services — not standard GP prescription. You'll need a GP referral to a specialist weight management clinic in your area. Eligibility criteria include: a BMI of 30 kg/m² or above, or BMI 27–29.9 with weight-related health conditions like type 2 diabetes, hypertension, or cardiovascular disease.

Adults of South Asian, Chinese, Black African, or Black Caribbean heritage may be eligible at lower BMI thresholds (from 27.5) due to higher health risks at those levels. The NHS consultation will assess your medical history, current medications, and suitability. However, NHS waiting lists are substantial — many regions have waiting times of 6–12 months or longer. Availability also varies significantly by ICB (Integrated Care Board) region.

Once approved, your dose will be titrated gradually, starting at 2.5 mg and increasing weekly over 16–20 weeks to reach your maintenance dose. Throughout treatment, you'll have regular follow-up consultations to monitor weight loss, side effects, and whether tirzepatide remains appropriate for you.

NHS vs Private Mounjaro Consultation Routes

The NHS route is free but involves waiting lists and regional variation in service availability. The private route offers quicker access: online consultations with UK-licensed prescribers, typically within days. A private consultation involves a detailed medical questionnaire and a virtual assessment with a doctor. If approved, your prescription is issued and dispensed by a GPhC-registered pharmacy. Private consultations cost £0–£150 depending on the provider, and tirzepatide itself costs approximately £129–£320 monthly depending on your dose.

Both routes require a proper clinical assessment — you cannot simply order tirzepatide online without a consultation. Whichever pathway you choose, your prescriber will discuss eligibility, expected weight loss (clinical trials showed an average of 15–clinically studied weight loss), potential side effects, and what happens if you need to stop the medication. Use our free GLP-1 weight loss calculator to estimate potential outcomes at different doses.

How We Built These Tools

Every calculator and guide on myweightcheck is built with accuracy and transparency in mind. Our BMI calculator uses the standard WHO formula with NHS-aligned category thresholds. Our GLP-1 projections are modelled on published clinical trial data from SURMOUNT and STEP studies.

All data stays on your device. We don't require sign-ups, and we don't store your health information on any server. Your progress tracker saves data locally in your browser using localStorage, so only you can see it.

We're based in the UK and our tools are designed specifically for UK users — supporting stone & lbs by default, referencing NHS guidelines, using NICE-approved medication data, and linking to GPhC-registered pharmacies.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Can I get Mounjaro through my NHS GP without a specialist referral?

No. Mounjaro is only prescribed through NHS specialist weight management services (tier 3 or tier 4), not by standard GPs. You'll need a referral from your GP to your local specialist service. However, you can access Mounjaro privately through an online consultation without an NHS referral.

What happens during an NHS Mounjaro consultation?

Your specialist will review your medical history, current medications, BMI, and any weight-related conditions. They'll assess whether tirzepatide is suitable, discuss the kwikpen injection device and dose titration schedule, and explain potential side effects. You'll also discuss lifestyle changes (diet and exercise) alongside medication.

How long are NHS waiting lists for Mounjaro?

Waiting times vary significantly by region. Many NHS specialist services have waiting lists of 6–12 months or longer. Some areas may have shorter or longer waits. Contact your local ICB to ask about current waiting times in your region.

What are the eligibility criteria for NHS Mounjaro?

You typically need a BMI of 30 or above, or 27–29.9 with weight-related health conditions (diabetes, hypertension, cardiovascular disease). South Asian, Chinese, Black African, and Black Caribbean adults may be eligible from BMI 27.5. Your GP and specialist will assess your individual circumstances.

How does private Mounjaro consultation differ from NHS?

Private consultations are faster (days vs months), available without NHS referral, and conducted online. You pay for the consultation (usually £0–£150) and medication (£129–£320 monthly). NHS consultations are free but involve waiting lists. Both require proper clinical assessment — neither offers unlicensed or unsupervised access.
