What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Clinical Trial Data and Mechanism Differences

Mounjaro (tirzepatide) activates two hormone pathways: GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1). In the SURMOUNT-1 clinical trial, participants on the maximum 15 mg dose lost an average of 20.9% of body weight over 72 weeks, alongside a reduced-calorie diet and increased physical activity. Those on 10 mg lost approximately 19.5%.

Wegovy (semaglutide) activates GLP-1 only. In the STEP-1 clinical trial, participants on the maximum 2.4 mg dose lost an average of 14.9% of body weight over 68 weeks under similar lifestyle conditions.

Eligibility, Prescribing Rules, and Individual Suitability

Both Mounjaro and Wegovy follow the same UK eligibility rules: adults with a BMI of 30 or above, or 27–29.9 with weight-related health conditions (type 2 diabetes, prediabetes, high blood pressure, dyslipidaemia, obstructive sleep apnoea, or cardiovascular disease history). Lower BMI thresholds apply for South Asian, Chinese, Black African, and Black Caribbean adults. NICE has approved both medications for use within NHS specialist weight management services.

Private prescribing is available through registered online clinics following clinical assessment. Prescribers assess factors including tolerability of gastrointestinal side effects, baseline health conditions, concomitant medications (oral contraceptives interact with both), and individual response patterns. Neither medication is "better" universally — suitability depends on your personal health profile, medical history, and prescriber judgement. What's the main difference between Mounjaro and Wegovy?
