What Is BMI and Why Does It Matter?
Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).
While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.
The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.
How Mounjaro Access and Costs Work in the UK
Mounjaro is available in the UK through two distinct pathways, each with different cost implications.
NHS Access: NICE has approved tirzepatide for use within NHS specialist weight management services (tier 3 and 4). However, NHS access is not automatic — you must be referred to a specialist service by your GP, and availability varies significantly by region. Waiting times can be substantial (often several months to over a year in many areas). When prescribed through the NHS, Mounjaro is provided free at the point of care, though prescriptions to pharmacies are subject to standard NHS prescription charges (currently £11.90 per item in England for those not exempt).
Private Access: Private prescribing is available through UK-licensed prescribers via online consultations. Private costs typically range from £129 to £320 per month depending on the dose required. The 2.5 mg starting dose is usually the least expensive; maximum doses (15 mg) are more costly. Each KwikPen contains four weekly doses. Private providers include regulated GPhC-registered pharmacies, ensuring quality and safety standards equivalent to NHS dispensing.

NHS Eligibility and Specialist Services
To access Mounjaro through the NHS, you must meet NICE-approved eligibility criteria: BMI of 30 or above, or 25–29.9 with at least one weight-related health condition (type 2 diabetes, cardiovascular disease, high blood pressure, sleep apnoea). For adults of South Asian, Chinese, Black African, or Black Caribbean heritage, lower BMI thresholds apply (from BMI 27.5). Your GP refers you to your local integrated care board's (ICB) specialist weight management service. From there, a multidisciplinary team assesses suitability. Use our free BMI calculator above to check whether you meet the BMI threshold for potential eligibility. Regional availability of NHS tirzepatide varies — some areas have it available, others do not yet offer it due to funding constraints.

Frequently Asked Questions
Common questions about BMI, weight loss medications, and our free UK tools.
What's the difference between NHS and private Mounjaro costs?
NHS prescriptions are free once dispensed (subject to standard £11.90 prescription charges in England, or free if exempt). Private costs range £129–£320/month depending on dose. NHS waiting times are longer but no upfront cost; private access is faster with immediate cost. Both require clinical assessment; both are prescription-only.
How long is the NHS waiting list for Mounjaro?
Waiting times vary significantly by region. Some areas have waiting lists of 6–12 months; others may be shorter or longer. Contact your local integrated care board (ICB) specialist weight management service for current waiting times in your area. Private routes typically offer faster access.
Can I get Mounjaro through my GP without specialist referral?
No. NICE guidance specifies that Mounjaro is prescribed through specialist tier 3 weight management services, not in primary care. Your GP must refer you to the specialist service in your area. Private prescribing bypasses this and is available directly through online consultation with a licensed prescriber.
Is Mounjaro cheaper privately or on the NHS?
On the NHS, Mounjaro is free once prescribed (except for standard prescription charges). Privately, costs are £129–£320/month. However, NHS waiting lists can be very long, and not all areas currently provide NHS tirzepatide. Private access is faster but comes with ongoing costs.
What dose of Mounjaro would be most expensive?
The maximum dose of 15 mg is typically the most expensive. Starting doses (2.5–5 mg) are least expensive. Costs vary by private provider. Your prescriber will recommend the appropriate dose based on your response and tolerability, starting at 2.5 mg and increasing every 4 weeks.
