What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

How Much Does Mounjaro Cost in the UK?

The cost of Mounjaro in the UK depends entirely on your access route. NHS prescriptions of Mounjaro carry only the standard prescription charge of £9.90 (or are free if you're eligible for free NHS prescriptions). However, NHS availability is extremely limited — Mounjaro is not routinely prescribed in primary care. Access is through specialist tier 3 or tier 4 weight management services, which have strict eligibility criteria and significant waiting lists in many regions.

Private prescribing costs typically range from £129 to £320 per month, depending on your dose. Each KwikPen contains 4 weeks' supply (4 weekly injections). Pricing varies by provider and dose level: the starting 2.5 mg dose is usually at the lower end, while the maximum 15 mg dose costs more. Some private providers offer discounts for upfront payment of multiple months.

  • NHS: £9.90 per prescription (if available through specialist services)
  • Private: approximately £129–£320 per month depending on dose
  • Total first-year private cost: roughly £1,548–£3,840

NHS Eligibility and Access Pathways

To access Mounjaro through the NHS, you must meet strict eligibility criteria and be referred to a specialist weight management service (tier 3). You must have a BMI of 30 or above, or BMI of 27–29.9 with at least one weight-related health condition such as type 2 diabetes, high blood pressure, or sleep apnoea. South Asian, Chinese, Black African, and Black Caribbean adults may be eligible at lower BMI thresholds (from 27.5).

Once referred, you'll wait to be assessed by a multidisciplinary team including dietitians, psychologists, and doctors. Waiting times vary dramatically by region — some areas have waiting lists of 6–12 months or longer. Use our free BMI calculator above to check if you might meet the initial eligibility threshold. Your GP can discuss referral options and explain what to expect in your area.

The NHS currently approves Mounjaro (tirzepatide) for weight management through NICE guidance, but availability remains patchy. Many NHS regions have not yet integrated it into their tier 3 services, or have extremely limited capacity. Private prescribing offers faster access — you can be assessed and receive a prescription within days through a GPhC-registered online pharmacy. Private costs are higher but delivery is quicker.

Many people use a hybrid approach: they register with a private prescriber initially while waiting for NHS assessment, then transition to NHS prescribing once they reach the top of the waiting list. This ensures continuous treatment. Both routes require a full clinical assessment to confirm you're medically suitable before a prescription is issued.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

Is Mounjaro covered by the NHS?

Yes, NICE has approved tirzepatide (Mounjaro) for weight management in the UK. However, NHS availability is limited to specialist tier 3 and tier 4 weight management services. Your GP can refer you, but waiting lists are lengthy in many regions. Once prescribed through the NHS, you pay only the standard prescription charge of £9.90 per prescription, regardless of dose.

How much does a Mounjaro injection cost privately?

Private Mounjaro costs typically range from £129 to £320 per month, depending on your dose. Each KwikPen contains 4 weekly injections. Starting doses (2.5–5 mg) are usually cheaper than maximum doses (15 mg). Over a year, private costs can range from approximately £1,500 to £3,800. Some providers offer discounts for upfront payment of multiple months.

What affects the price of Mounjaro in the UK?

The main cost factors are: your dose level (higher doses cost more), your prescribing route (NHS vs private), and the private provider (prices vary). Some online pharmacies offer price matching or bundled discounts. Your clinical assessment may also determine which dose you start at, which influences overall cost.

How long are NHS waiting lists for Mounjaro?

NHS waiting times for weight management services vary significantly by region, from 3–6 months in some areas to over 12 months in others. You can ask your GP what the current waiting time is in your ICB (Integrated Care Board). Private prescribing has no waiting list — assessment and prescription are usually completed within 1–2 working days.

Is private Mounjaro more expensive than NHS?

Yes — private prescribing costs significantly more (£129–£320/month) compared to the standard £9.90 NHS prescription charge. However, private access is immediate and doesn't require a lengthy waiting list. Some people choose private initially to start treatment quickly, then switch to NHS once their waiting list spot arrives, then transition back to private if they prefer convenience.
