Understanding Weight and BMI — NHS Guidance for UK Adults

Your weight and BMI are important health indicators, but understanding what they mean is crucial. The NHS uses specific BMI categories and thresholds to help you assess whether your weight falls within a healthy range for your height. This guide explains the NHS approach to weight management, how to interpret your BMI, and what support is available through the NHS if you're concerned about your weight.

What Is BMI and Why Does It Matter?

Body Mass Index (BMI) is the most widely used screening tool for assessing whether your weight falls within a healthy range. Used by the NHS, GPs, and weight management services across the UK, BMI provides a quick snapshot of your weight relative to your height. The formula is simple: your weight in kilograms divided by your height in metres squared (kg/m²).

While BMI isn't a perfect measure — it doesn't account for muscle mass, bone density, or fat distribution — it remains the standard first step in identifying potential weight-related health risks including type 2 diabetes, cardiovascular disease, high blood pressure, and certain cancers.

Below 18.5Underweight
18.5 – 24.9Healthy
25 – 29.9Overweight
30+Obese

The NHS also recommends adjusted thresholds for people of South Asian, Chinese, or Black African heritage — a BMI of 23 or above is classified as overweight, and 27.5 as obese. Our calculator includes this adjustment as a one-click toggle.

NHS BMI Categories and Healthy Weight Ranges

The NHS defines healthy weight using Body Mass Index (BMI), which is calculated using your height and weight. The bmi formula is: weight (kg) divided by height (m) squared. According to NHS guidance, the standard bmi range uk categories are:

  • Healthy weight: BMI 18.5 to 24.9
  • Overweight: BMI 25.0 to 29.9
  • Obese (Class I): BMI 30.0 to 34.9
  • Obese (Class II): BMI 35.0 to 39.9
  • Severely obese (Class III): BMI 40.0 and above

However, the NHS acknowledges bmi limitations — BMI does not distinguish between muscle and fat, so athletes may have a high BMI despite low body fat. For adults of South Asian, Chinese, Black African, or Black Caribbean heritage, the NHS recommends lower thresholds: a healthy bmi for these groups starts at 18.5 to 22.9. Your GP can help interpret what your BMI means for your individual health.

NHS Weight Management Pathway and Support Available

The NHS provides a structured approach to weight management across four tiers. Tier 1 includes self-help resources, digital tools, and community programmes. Tier 2 involves primary care support from your GP or practice nurse, including behaviour change programmes like the NHS Weight Loss Plan. Tier 3 offers specialist multidisciplinary weight management services, where prescription weight loss medications may be considered for eligible adults. Tier 4 is bariatric surgery, available for severe obesity with significant health complications.

If you're concerned about your weight, start by speaking to your GP, who can assess your individual health and recommend the most appropriate support. Use our free BMI calculator to check your personal BMI, and our weight converter to understand your weight in different units.

Frequently Asked Questions

Common questions about BMI, weight loss medications, and our free UK tools.

What is a healthy BMI according to the NHS?

The NHS defines a healthy BMI as 18.5 to 24.9. However, if you're of South Asian, Chinese, Black African, or Black Caribbean heritage, the NHS recommends a lower threshold of 18.5 to 22.9 because these groups face higher health risks at lower BMI levels. Your GP can discuss what's healthy for you individually.

What does the NHS recommend if my BMI is over 25?

If your BMI is 25 or above, the NHS recommends lifestyle changes including a reduced-calorie diet, increased physical activity, and behaviour change support. Depending on your BMI and any weight-related health conditions, your GP may refer you to a tier 2 or tier 3 weight management service for additional support or prescription medication.

Can I access weight loss medication through the NHS?

Yes, but availability varies by region. You'll need a referral from your GP to a specialist tier 3 weight management service. Eligibility typically requires a BMI of 30 or above, or 27–29.9 with weight-related health conditions such as type 2 diabetes, high blood pressure, or sleep apnoea.

Are there limitations to using BMI as a health measure?

Yes. BMI doesn't distinguish between muscle and fat, so very muscular people may have a high BMI despite low body fat. It also doesn't account for where fat is stored on your body. The NHS recommends looking at BMI alongside waist circumference and other health markers for a fuller picture.

When should I speak to my GP about my weight?

It's worth speaking to your GP about your weight if you're concerned about your health, have recently gained significant weight, have weight-related health conditions, or want professional guidance on managing your weight safely. Your GP can assess your individual situation and recommend appropriate support.
